Veterans Health

Receive care from a military veteran who understands the experiences and healthcare concerns that can follow military service. Services include preventive care, chronic-condition management, health evaluations, medication review, care coordination and independent medical opinions for qualifying cases.

Primary Care

Establish care with a local provider who understands your complete health history. Services include preventive visits, wellness exams, chronic-condition management, laboratory testing, blood work, medication management and care coordination for patients age 6 and older.

Geriatric Care

Personalized primary care helps older adults manage changing health needs while maintaining independence and quality of life. Services may include chronic-condition monitoring, medication review, preventive care, mobility and fall-risk assessment, laboratory testing and caregiver collaboration.

Shockwave Therapy

Shockwave therapy is a noninvasive treatment option that may be considered for selected chronic musculoskeletal conditions. The clinic also provides consultations concerning low-intensity shockwave therapy for erectile dysfunction. Candidacy is determined through an individual evaluation.

Ebony Maynard, APRN-FNP is a military veteran and former law enforcement officer with a personal commitment to serving veterans, first responders and their families. At EM Healthcare & Associates, veterans receive respectful, individualized care focused on their current health, medical history and long-term quality of life. Our services include primary care, chronic-condition management, preventive health evaluations and care coordination.

Nexus Letter services are available separately for qualifying veterans who need an independent medical opinion related to a VA disability claim. Nexus Letters do not guarantee claim approval, and all final benefit decisions are made by the U.S. Department of Veterans Affairs.

Ebony Maynard is a Family Nurse Practitioner, military veteran and founder of EM Healthcare & Associates. She brings more than 13 years of healthcare experience across family health, neurology, intensive care, rehabilitation, long-term care and military healthcare settings.

Her approach centers on listening carefully, explaining options clearly and developing realistic care plans that reflect each patient’s health history, lifestyle and goals.
