Military and First Responder Discount Policy

Who Is Eligible?

Active-Duty Military Members

Current members of the United States:

• Army
• Navy
• Air Force
• Marine Corps
• Coast Guard
• Space Force

A valid active-duty military identification card or other acceptable proof of current service may be required.

United States Military Veterans

Honorably discharged veterans of the United States Armed Forces may qualify. Acceptable documentation may include:

• Veteran Health Identification Card
• Veteran ID Card
• DD Form 214
• Military retiree identification
• Other documentation accepted by the clinic

First Responders

Eligible first responders may include currently serving:

• Law enforcement officers
• Firefighters
• Emergency medical technicians
• Paramedics
• Other qualifying emergency response personnel

A current department-issued identification card, badge or other proof of service may be required.

Qualifying Immediate Family Members

The discount may also be extended to:

• Spouses of eligible individuals
• Dependent children younger than 18

The clinic may request documentation confirming the family relationship.

Discount Amount

Eligible patients may receive 10% off the clinic’s standard self-pay price for qualifying services.

Not every service, product or charge is eligible. Please confirm eligibility and the applicable self-pay price with the clinic before scheduling or receiving a service.

Eligible Self-Pay Services

The discount may be applied only to services that:

• Are designated by EM Healthcare & Associates as discount-eligible
• Are provided and billed directly by the clinic
• Are paid entirely by the patient
• Are not submitted to an insurance carrier
• Are not subject to an insurance contract or other restriction that prevents the discount

Qualifying services may change. Please contact the office before your appointment to confirm whether the service you are considering is eligible.

Insurance Cost-Sharing Is Not Discounted

The military and first responder discount does not apply to any service submitted to health insurance.

The discount cannot be used to reduce or waive:

• Copayments
• Deductibles
• Coinsurance
• Insurance-assigned patient responsibility
• Noncovered balances associated with an insurance claim
• Charges remaining after an insurer processes a claim

If any portion of a service is billed or submitted to an insurance carrier, the discount cannot be applied to that service or its remaining balance.

Patients may not combine insurance billing with the self-pay discount for the same service.

Additional Exclusions

Unless the clinic provides written confirmation otherwise, the discount does not apply to:

• Laboratory services billed by an outside laboratory
• Imaging or diagnostic services from outside facilities
• Specialist or third-party professional fees
• Prescription medications
• Retail products
• Medical supplies
• Supplements
• Services already subject to another discount or promotion
• Insurance-billed services
• Insurance copayments, deductibles or coinsurance

The discount has no cash value and cannot be exchanged for cash, transferred to another person or applied retroactively to previous charges.

How to Request the Discount

1. Tell the Clinic Before Your Visit

   Inform the office that you would like to use the military or first responder discount when scheduling or before services are provided.

2. Provide Proof of Eligibility

   Present an accepted military, veteran or first responder identification document. Qualifying family members may be asked to provide proof of their relationship to the eligible individual.

3. Confirm That the Service Qualifies

   Clinic staff will confirm whether the planned service is eligible and provide the applicable self-pay price.

4. Pay the Self-Pay Balance

   The discounted balance is due according to the clinic’s self-pay payment policy. The service will not be submitted to insurance.

Additional Policy Terms

• The discount cannot be combined with another promotion, package discount or special offer.
• Eligibility must be verified before the discount is applied.
• The discount applies only to designated services provided directly by EM Healthcare & Associates.
• Previous services and balances are not eligible for retroactive adjustment.
• The clinic may modify or discontinue this policy at any time.
• Application of the discount remains subject to applicable laws, payer agreements and clinic policies.
